Why are contactless forms recommended?
Fight The Spread of Covid-19
By using the system, visitors fill up the form via their own mobile device. This practice reduces physical interaction by avoiding touching of surface (of a pen and a log book), which then minimizes the risk of spreading Covid-19.
Facilitate Contact Tracing
The system keeps a record of all submitted data in the database, which is retrievable at anytime. These data are also useful to health authorities to identify and trace the people that might be infected with Covid-19.

How do I get the health forms?
Login to your Virtual Office account. Go to BizApp, and then select Health & Visitor Management. You will see three main sections on the landing page: Employees Form, Visitors Form, Delivery & Dispatch Form.
What is the Employees Form and how do I use it?
The Employees Form is the online form for your employees to record their health statuses upon entering the workplace. You can copy the URL link of the form and distribute it to your employees to perform daily recording. It also comes with a QR code where you can print out.
What is the Visitors Form and how do I use it?
The Visitors Form is for your guests who have a visiting purpose and a person to meet - be it your business partners, vendors, clients or even your job candidates - to submit their contact details and health statuses upon accessing your workplace. This form is accessible via a web URL or a QR code. For your visitor's quick and convenient access, you can print the QR code along with the URL and place it somewhere visible to your visitors so that they can access the form using their own mobile devices.
What is the Delivery & Dispatch Form and how do I use it?
The Delivery & Dispatch Form is an online form that dispatchers, postmen and delivery personnel can use to provide their contact details and declare their health statuses before entering your business premises. Similar to the other two forms, this form comes with a QR code as well. You can print this QR code and paste it on your office entrance so that the delivery personnel can quickly declare health status before dropping or collecting any parcels.
How do I get the QR code of a health form?
To get the QR code of a selected health form, click on the 'Get QR Code' link within its section. You can print this QR code and display it on a visible corner so that specific individual can access to the forms by scanning the QR code.
Can I put my company logo on the forms?
Logo customization is only allowed for paid version.
Is there a limit on how many records the system can keep?
The free version allows you to keep up to 1000 records, while the paid version allows saving unlimited records.
How do I define departments for the Employees Form?
At the Employees Form, select 'Configure Departments'. Then, enter the teams or departments into the given box and click 'Update' to save them. Please separate multiple departments in new lines.
Can I download the form records?
Yes. You can download the records in comma separated values format (CSV).
How do I view the submitted health records?
Click on the 'View Records' link to see the records.
